How to become a member?

Step 2: Take a Screenshot of your payment, showing the transaction ID.
Step 3: Fill up the membership form below and upload the documents as required along with the screenshot of your payment.
IMPORTANT NOTE:
- Our Team shall verify all your details and then issue an online ‘Membership Certificate’
- Please note that the Free Smile Foundation reserves the right to refuse membership at its own discretion. In all such cases, the decision of Free Smile Foundation shall be final.
- Incase of refusal of membership, the membership fess paid by you shall be refunded in full.
